Job Summary
As a Care Manager RN, assess and organize patient care through treatment stages, following guidelines. Manage benefits, oversee services, act as a case manager and trainer to achieve optimal results. Effective care management helps minimize unnecessary resource use.
Location: Temple, TX
Setting: Case Management
Schedule: PRN Weekdays and weekends needed
Essential Functions of the Role
* Conduct thorough assessment, implement care plan, collaborate with team for evidence-based high standard of care.
* Create patient care plans that focus on interventions, treatment goals, and self-management for the patient's benefit.
* Examine the health needs of patients and families, then provide necessary care to meet those needs.
* Assist patients with self-management using education, personal visits, and telephone interactions, inspiring patients to stick to their care plans.
* Research resources for patient needs; coordinate programs for correct service delivery and provide referrals to help patients.
* Be proactive in identifying problems and solutions in coordinating care to provide the best possible options.
* Act as a bridge between healthcare providers to ensure smooth communication and coordination among different facilities.
Key Success Factors
* A comprehensive knowledge of care management, along with efficient resource and utilization management.
* Proficiency in patient assessment and care management.
* Exceptional communication skills, both verbal and written, are crucial.
* The ability to keenly observe, assess, and document patient progress in relation to a care plan.
* The aptitude to assist patients with accessing community resources.
* The willingness and aptitude to work in harmony with both internal medical staff and external partners or organizations.
* Adaptive to change and able to comfortably navigate through a crisis environment.
* Exceptional discretion and respect for confidentiality as it pertains to all patient information, in accordance with policies and regulations.

Qualifications
* A minimum of an Associate's degree is required.
* The applicant should have at least two years of professional experience.
* The applicant should be a licensed Registered Nurse.
